List every participant on its own line in this format: Name — Age — T-shirt size. Ages are important for food planning — keiki ages 5 and under receive a smaller kid’s portion instead of the full Moa Ono chicken plate.
Photo & Video Release
I grant permission to The Courage of Esther 4:14, Team Jesus Hawai‘i, Guiding Light Ministries, and their representatives to use, reproduce, and publish photographs and video footage taken of me and any minor participants under my care during the Neal Blaisdell Park Prayer Walk. I understand these images may be used in publications, websites, social media, and promotional materials, without compensation, in perpetuity.
Liability Release & Waiver
I acknowledge that participation in the Prayer Walk is voluntary and may involve physical activity. In consideration of being permitted to participate, I — on behalf of myself, my heirs, and any minors under my care — release, waive, and hold harmless The Courage of Esther 4:14, Team Jesus Hawai‘i, Guiding Light Ministries, the City and County of Honolulu, and all event organizers, sponsors, and volunteers from any and all claims, demands, or causes of action arising from participation, including injury, illness, loss, or damage to property. I confirm that I and all participants registered above are physically able to take part.
After submitting, we will follow up by email with payment details and any final logistics. Please bring your own chairs / beach mats. Mahalo!
